We Accept Referrals For:

Participants of any age with a neurological or progressive condition where MAXIN’s specialist focus is appropriate.

NDIS participants who are Plan‐Managed and have Support Coordination in place (Level 2 or Specialist).

Participants living in Greater Geelong, and selected Melbourne referrals, where clinically appropriate and feasible.

Information to Include in Your Referral:

Participant details and preferred contact information.

NDIS number, current plan dates, and confirmation of Plan Management and Support Coordination.

Relevant diagnoses, current living situation, and key risks or concerns.

The reason for referral and the outcome being sought.

Existing reports or key clinical documentation, where available.

What Happens Next?

Once received, MAXIN will review the referral information, confirm suitability, and advise of likely
timeframes for commencement. Further information may be requested to support safe and appropriate service delivery.
